Police are concerned for a mother Tiegan Carr who fled Victorian town of Wangaratta into NSW

A desperate search is underway for a young mother who disappeared with her toddler and baby more than a month ago.

Tiegan Carr was staying in the rural Victorian town of Wangaratta, just south of the NSW border, with her two children up until early July.

Soon after the 26-year-old fled the town and took her three-year-old daughter Kelahni and two-month-old baby Freya with her.  

Ms Carr has had limited contact with authorities but police believe she has crossed state lines and is now staying in Albury, an hour north-east on the New South Wales border. 

Victoria Police said they hold concerned for the the welfare of the young mother and her two children.

It is understood the Ms Carr is also dealing with a medical condition.

Police have urged anyone who has seen the family or has any information on their current location to ring Bendigo Police Station on (03) 5448 1300.
